We need to complete these annually for every young person who comes to Cell Youth Group either at the start of the academic year or when someone starts coming to the group What you need to know….
- the information you provide is important and enables us to communicate well, ensure good practice, keep in touch and know who to contact in case of emergency.
- this information is held securely (in accordance with the Data Protection Act 1998).
If any information or details change please let the Cell Leaders know as soon as possible.
Before completing the online consent form please read the following:
By completing this consent form I agree to the following and I give permission for my child to attend Cell and participate in the youth group.
I give permission for my child to take part in normal weekly activities plus the following (if they are arranged and fit into the programme); Send the Fire events, other one off events or trips (e.g. cinema, 10-pin bowling, walks), which could include Bonfire Night Celebrations on the recreation ground in Chipping Campden, attendance at The Cotswold Olympicks on Dover’s Hill then back at the church for the Late Night Cafe, helping in local community initiatives (such as practical tasks in people’s homes and gardens – note this will always be with and under the supervision of the Youth Leaders).
If I am unable to provide transport or arrange lifts for my child to attend the group, I give permission for my child to have a lift in a leader’s vehicle (providing they hold a full driving licence and insurance) either to or from the group.
As a Christian based group we want everyone to enjoy the time we spend together. As such we don’t permit bullying, intimidating or threatening behaviour either to leaders or other young people.
I understand that the leaders will take all reasonable care in looking after my child but the leaders cannot be held responsible for any loss or damage to property or belongings. Furthermore, I agree to reimburse the leaders/church if my child causes any malicious/accidental damage to other people’s property.
